Thank you for your interest in Desert CHEA. All DCHEA members must subscribe to our Statement of Faith and agree to abide by our Code of Conduct. Please go over this information with your spouse and your children prior to submitting this application. See the "Term of Service" link below.

Qualifications for membership:

Members have expressed a personal faith in Jesus Christ as their Lord and Savior and as the only means of forgiveness for our sins.
Members can express full acknowledgment and agreement with the Statment of Faith that DCHEA and CHEA have endorsed.
Members must be homeschooling their children in one of two ways:
- By filing a Private School Affidavit with the CDE.
- By enrolling in a Private School Satellite Program (PSP) who file a PSA on your behalf.
Members have read and agree to abide by our Code of Conduct.

Submitting an application and payment does not automatically qualify you for membership. All membership applications are reviewed prior to receiving final approval. We will contact you in the next few days to discuss your application.

I hereby acknowledge that it is my desire for my child to participate in Desert Christian Home Educators Association-sponsored activities, including activities at Grace Chapel in Indio and/or off Church premises as well as transportation to and from activities.

I am (my child is) voluntarily participating in these activities, including transportation to and from such activities with knowledge of the dangers involved, and hereby agree to accept any and all risks of injury as a result of such participation and transportation.

As lawful consideration for permitting me (my child) to participate in such activities, including the transportation to and from such activities, I hereby release Desert Christian Home Educators Association, Grace Chapel, their officers, agents, and members of the Board of Directors and of the Board of Elders from all action, claims, or demands I or my heirs, distributes, Guardians, legal representative or assigns now have may hereafter have for any injuries or damages resulting from the agents of the Board of Directors or the Board of Elders before or during my participation in such DCHEA sponsored activities on or away from the church premises, including transportation to and from activities.

I have carefully read this agreement and fully understand its content. I am aware that this is a release of liability and assumption of risk, I am agreeing to it of my own free will.

This Consent and Release Form liability shall remain effective until it is revoked in writing and delivered to any member of the Board of Directors.
